I hereby release Outlier Tattoo and its employees, agents and contractors from all manner and type of liability, claim, action, demand and compensation, in law and in equity, which I or my heirs have or might have, now or hereafter, by any reason of my request to be tattooed. I realize that misrepresentation or falsification of information provided by me is a crime and subject to prosecution. I further certify that I am an adult over the age of eighteen(18) and am not intoxicated or under the influence of any drug, narcotic or substance and make these statements entirely of free will and sound mind. I also agree to follow the procedures outlined for proper care and healing of my tattoo. This is a legal binding contract. You are being tattooed by a highly experienced professional, in a clean environment using sterile equipment and supplies including the highest quality pigments available. Every care is taken during the application of your tattoo. The healing process directly effects the end result and longevity of your tattoo. Please follow all care instructions on the aftercare sheet.

I acknowledge by signing this agreement that I have been given the full opportunity to ask any and all questions which I might have about the obtaining of a tattoo and that all of my questions have been answered to my full satisfaction. I specifically acknowledge that I have been advised of the facts and matters set forth below and agree as follows: I have told my artist if I have Diabetes, Epilepsy, Hepatitis, Hemophilia, HIV-AIDS, or any other communicable disease, hearth condition, etc., or take medicine which thins the blood. I have advised my artist that I am NOT pregnant or nursing. I am NOT under the influence of alcohol or drugs. I have advised the artist of any latex or medical tape allergy.
